Part 1 - Form Setup
1.) Add Form Fields - For more general information on building forms please see the following article here.

2.) Name your form - i.e. DataGrid: Business Search (while the Field Keys in the form must match the table above exactly, the form title can be whatever you'd like to call it)
3.) Turn on Custom Form Responses
4.) Click on the “Publish” button and confirm
Part 2 - Add-On Configuration
1.) Navigate to the Marketplace under "Add-Ons". (Must have admin access)
2.) Select the "DataGrid - Business" Add-On Card
3.) Navigate to the "Configuration" tab and click "Enable"
4.) Select the Published DataGrid: Business Form and Select the Default Lead Status For Pins
5.) Click "Finish"
You're all set! Next you'll need to drop a pin and submit a form to begin populating businesses nearby as pins. For more information on this please see the following video below:
